Footprints on sand: First-ever ankle replacement surgery in Dubai
“Using this kit and thanks to the team at MOSH, many patients can now get this surgery done here, which saves them from expensive treatment abroad, which costs the health authorities more than AED4 billion a year,” Says Dr. Taba.
The UAE has always positioned itself as a leader in the advancement of the medical and now it adds another first to its achievements, having hosted the first Total Ankle Arthroplasty (TAA) or ankle replacement surgery by a resident surgeon of the UAE. The first of its kind surgery was conducted by Dr. Ma’an Taba of Medcare Orthopaedics & Spine Hospital (MOSH) on a osteoarthritis patient living in Dubai.
Dr. Taba, Consultant orthopedic foot and ankle surgeon at MOSH, led the Dubai-based team of specialists who are at the forefront of medical research in TAA and who have revolutionized this type of treatment, taking ankle replacement surgery beyond the traditional ‘ankle fusion’ procedure which has been the de facto treatment for worn-off joints, severe fractures, rheumatoid arthritis or osteoarthritis. While TAA uses artificial ankles designed to mirror the movements of the ankle joint, ‘ankle fusion’ is more invasive as surgeons use pins, incisions and bone grafts limiting patient activities in the future. TAA has thus become the preferred choice of orthopedic surgeons, specialists and orthopedic consultants around the world as the preferred treatment for such patients as those with end-stage ankle osteoarthritis. Essential to the success of Dr. Taba’s surgery was an ideal candidate and the use of the highest quality prosthesis kit available in the industry.
The patient was a 55-year old male with osteoarthritis who works as a manager with a relatively active job and requires the utmost mobility. Suffering for years with osteoarthritis, the patient refused ankle fusion, but chose TAA based on Dr. Taba’s renowned expertise in foot and ankle surgery and the availability of a special prosthesis kit which is found exclusively at MOSH.
For this procedure, the doctor used a three-component prosthesis kit that includes a mobile bearing with an anatomical design and high resistant polyethylene that can tolerate more than 10 million cycles without getting worn.
The patient was an ideal candidate given his lifestyle and general health (no diabetes, non-smoker) which helped qualify him for surgery. Due to the complexity and fragility of the procedure, there were many filters and restrictions that the patient had to undergo to qualify for TAA treatment. The surgery took a little over three hours and the patient was discharged the next day with follow up therapy after three weeks and complete recovery in six weeks.
“The ankle joint is like the knee and hip; it is extremely important and needs to be treated with tremendous respect and care, especially in cases of rheumatoid disease and arthritis. We had to be very careful since the long term success of this surgery depends completely on the candidate as well as the procedure,” explains Dr. Taba. “Every step of the treatment has an impact on successful replacement and recovery, from the incision in the tibia to the preparation of soft tissue and ligaments, the alignment of the hind foot, right down to the corrective surgery to make minor adjustments using minimally-invasive techniques.”
